NEW PATIENT FORMS
First Visit Case History

At your first visit, we will gladly help you fill out your Chiropractic Case History/Patient Information per your request as well as the Symptom Survey Form. Or, download here, fill it out at home, and bring it to LaBlanc Chiropractic on your first visit. Either way, please be as specific as possible when filling out this form. Since chiropractic care can and often should be a part of a much larger lifestyle change, Our Symptom Survey Form is the precursor to our nutritional therapy. Along with your adjustments and other forms of physical therapy and rehabilitation, we believe that taking better care of yourself through proper diet and exercise is critical to your overall health. If you share this commitment, we encourage you to take the time to answer questions about your general health habits in addition to your case history.

WHAT IS CHIROPRACTIC? WHY CHIROPRACTIC?
Chiropractic Explained

Chiropractic is an excellent, alternative healthcare option that focuses on the body’s natural ability to correct and take care of itself without surgery or prescription medication.

Chiropractors are the second largest providers of healthcare, second only to medical doctors, and share much of the same educational background and certification as medical doctors. In fact, chiropractic doctors and medical doctors often work together through patient referrals. Your medical doctor may recommend chiropractic care if you do not need surgery or drugs – or as a conservative option before considering surgery. In turn, a chiropractor may refer you to medical specialists including neurologists or radiologists, for example, to complement your chiropractic treatment plan or when recommending lab work or further testing.

Chiropractic treatments can include adjustments and other forms of physical therapy and rehabilitation. There is also a growing focus on health and lifestyle counseling, which does include the use of nutritional supplements.

Adjustments of the spine – and the joints in extremities, including the neck, shoulders, arms, hands, and legs – are sometimes referred to as manipulations and are gentle, manual thrusts that move the vertebrae or joint back to its normal position.

Spinal adjustments may relieve pain associated with:
• Headaches & certain types of migraines
• Menstrual cramps
• Allergies
• Asthma
• Stomach disorders
• Dysfunction of the neck or lower back (that causes arm, hand, or leg pain), including dysfunction typically caused by personal injury (i.e., car accidents or work-related injuries)
